Pathways Case Management Services provides Targeted Case Management (TCM) for individuals with intellectual and developmental disabilities throughout Kansas. Our mission is to empower individuals to achieve their goals and live fulfilling, independent lives within their communities.

We take a person-centered approach to service coordination, focusing on each individual's unique strengths, preferences, and aspirations. Our experienced case managers work collaboratively with individuals, families, and service providers to ensure access to the resources and support needed for success.

As a licensed and certified provider in Kansas, we are committed to maintaining the highest standards of quality and professionalism in all aspects of our service delivery. We believe in building lasting relationships based on trust, respect, and genuine care for the people we serve.

What is Targeted Case Management?

An ongoing process for the identification of the person's needs, the determination of a person's preferred lifestyle, and the resources that are available to the person, through both formal and informal evaluation methods.
This process involves the participation of the person and the person's support network to develop or assist in the development of updating and reviewing the person's PCSP (Person Centered Support Plan) and any related service or support plan after a comprehensive assessment of the individual's support needs. These plans are developed to assist the person/circle of support in meeting the person's needs and achieving the person's preferred lifestyle. Support planning includes providing assistance to the person in being knowledgeable about the types and availability of community services and support options, in receiving information regarding the rights of persons served pursuant to the developmental disabilities reform act and implementing regulations, the content of which shall be approved by the commission, and in obtaining the community services and supports of the person's choice.
Arranging for and securing supports outlined in the person's person-centered support plan. This service includes developing and accessing natural supports and generic community support systems, including pursuing means for gaining access to needed services and entitlements, and seeking modification of service systems when necessary to increase the accessibility to those systems by the person.
Including ongoing activities that are necessary to ensure that the person-centered support plan and related supports and services are effectively implemented and adequately addressing the needs of the person.
Including the planning of and arranging for services to follow the person when the person moves between any of the following:
  • From school to the adult world
  • From an institution to community alternatives
  • From one kind of service setting to another kind of service setting
  • From one provider to another provider
  • From one service area to another service area

Working with Pathways

No. TCM services are covered by Medicaid (Title XIX) at no cost to you or your family. We are contracted with Healthy Blue, United Healthcare, and Sunflower Health Plan. If you are not yet Medicaid-eligible, private pay arrangements are available.
If you are already established with your local CDDO, contact us at rschultz@pathwayscms.com or through our website. We'll discuss your needs and our approach to see if we're a good fit. You'll then complete a choice form with your CDDO indicating Pathways as your selected provider. Once we receive notification of your choice form, we will contact you to schedule your first visit.
Yes. If you're currently receiving TCM services from another agency and would like to transfer to Pathways, we can help facilitate that transition. Contact us and we'll guide you through the simple process of changing providers.
Your relationship with your case manager is important. While we work hard to ensure excellent matches, if concerns arise, please contact us immediately. We're committed to resolving any issues and ensuring you receive the quality support you deserve.
We prioritize responsive communication. Your case manager will provide you with their contact information and will respond to non-emergency concerns within one business day. For urgent situations, we handle TCM Crisis Funding Requests promptly and will work with you to address immediate needs.

We take your privacy seriously. All our staff are trained in HIPAA compliance and follow strict confidentiality protocols. Your personal and health information is stored securely and only shared with your explicit consent or as required by law to ensure your safety.
Yes. All our case managers must meet guidelines set by Kansas Article 63. We prioritize candidates who hold Bachelor's degrees in social work, psychology, special education, or related human services fields, and we value lived experience supporting the I/DD community. All case managers complete comprehensive training including Kansas TCM certification, person-centered planning, HIPAA compliance, and ongoing professional development. Our staff undergo thorough background checks including criminal history, abuse/neglect registries, and motor vehicle checks per Article 63 guidelines.
Our case managers are mandated reporters, meaning they are legally required to report any suspected abuse, neglect, or exploitation to the appropriate authorities. We believe in mutual respect and transparency—unless there is an imminent safety concern to you, your loved one, or our staff, you will be notified that Pathways has made a report. We believe in building long-lasting, trusting relationships. Your safety and wellbeing are our highest priorities.
No. Your case manager will travel to meet you at the location of your choice using their own transportation. Pathways does not provide transportation to clients but can help connect you to community resources.
Life happens! Just contact your case manager as soon as possible—we ask for at least 24-48 hours notice when possible so we can adjust schedules. We understand that unexpected situations arise and will work with you to find a new time that works.
Yes! Connecting you with resources and services is a core part of case management. Your case manager can help you explore housing options, employment supports, therapy services, recreational activities, and many other community resources based on your individual goals and needs.
Your annual Person-Centered Support Plan (PCSP) meeting is an opportunity to review your progress over the past year, celebrate achievements, discuss what's working well and what isn't, and set new goals for the year ahead. You decide who you want to be there—family members, friends, service providers, or anyone important to you. Your case manager typically facilitates the meeting, but you're in charge of the conversation and decisions.
